The Athletic Manual for Parents and Students has information you will find helpful about the J.P. Case Athletic program. All necessary Health forms you must submit in order for your child to participate in Athletics are also found in the manual. 
    Included in the manual are the following forms:
    Pre-Participation Sports Physical Evaluation Packet:  Forms are to be fully completed, especially vision, BP and pulse, height, and weight, by examining licensed provider within 365 days prior to the first practice session.  If vision is corrected, the primary provider needs to screen vision with lenses.   If a student sees an eye doctor, results of a recent eye exam must be attached.  Stating “sees eye doctor” is not acceptable. Physical examinations are good for one calendar year.  

    Health History Update Questionnaire Packet:  Forms are to be completed by parent or guardian.  These forms are required for each sport season and must be reviewed by examining provider only at the time of a physical exam. Health History Update Questionnaire forms must be signed and dated within 90 days prior to the first practice session. 

    Any student listing on the Health History form as having asthma (Asthma Treatment Plan, allergies (Allergy Action Plan) or daily medications (Doctor’s orders), must have the appropriate forms filed in the Health Office.

    Concussion Policy/Cardiac/Eye Injury Form: Cardiac brochure, concussion and eye injury information sheet must be read.  The "Parent/Student Cardiac, Concussion & Eye Injury Acknowledgement Sheet" must then be signed and dated by both student and parent/guardian. (Included in both packets and must be completed for each sport).  

    Any student that does not have this information on file and is not cleared by the Health Office CANNOT try-out until the information is attained.  All forms that are submitted AFTER the acceptance date (for each season) WILL NOT BE ACCEPTED.  Health History forms must be handed in prior to the start of tryouts, regardless of grade level. NO FORMS WILL BE ACCEPTED AFTER THE DEADLINE.


    Physical examinations are good for one calendar year. All incomplete forms will be returned. It is suggested that you make copies of all forms.
