Incoming 5th & 6th Graders - Documents Needed by the Health Office

Dear RFIS Parents/Guardians: 

Here is some information to help you prepare for the next school year. 

1.  Medication:  Current students all medication will be sent home with your child unless you have heard from the Health Office.  

2.  All medication orders (even ones that were recently submitted) expire on June 30, 2024. New order forms will be required by August 15, 2024. Please visit our website for the forms

Your doctor’s office may use their own template for treatment/action plans. The district has permission to administer medication forms for parents/ guardians to sign. 

3.  It is the parent’s responsibility to provide necessary medication (inhalers, EpiPens, benadryl, etc.) before the first day of school, in the originally labeled box. Check the expiration date. 

4. All students entering 6th grade must have a Tdap booster and  Meningococcal vaccine after their 11th birthday: If your child’s birthday is after the start of school please notify the health office of their appointment date. Please discuss HPV vaccines at this time as recommended by the CDC.  

5.  Physicals - every student should have a yearly physical, but it is required for students new to the NJ Public School System and those wanting to participate in school sports.
