High School Health Office Information


New Jersey State law requires that each student must be examined at their medical home (family doctor) upon entry into a school district. Therefore, Hunterdon Central requires a physical examination for all Grade 9 students, as well as new students in any grade level.

In addition, one physical per year is required for students participating in Athletics. Please note that a sports physical will fulfill the requirement for a freshman physical or a new student physical. Information on athletic form due dates and procedures can be found on Central's Athletic website. 

Forms can be picked up in Hunterdon Central Health Offices or found online on the Health Services website.

Grade 9 and new student physicals must be turned into Health Offices by September 30, 2019. PLEASE NOTE:  any students who have not submitted a Grade 9/new student physical to Health Offices by the due date will not be allowed to return to school until the physical has been completed and submitted.
